  • When calling or emailing about care for a new patient, include:

    • Your pet's name, age, gender, species (e.g. cat, dog, etc.), and breed

    • Your name, address/approximate location, email address, and phone number

  • Please leave a detailed message if we are unable to answer.  

    • We are often in an appointment and so unable to answer the phone.

    • We make every effort to return all calls & emails within 1 business day.

  • For more pressing concerns, please call.


Clinic Hours:  Monday-Friday, 9AM-5pm

Due to COVID-19, we have limited appointment availability and it may take longer than usual for us to respond to calls and emails. 

  • Please call and/or email for appointment availability.

    • Evening and weekend appointments may be available if set up in advance.

    • Emergency & after hours communications & appointments are generally reserved for current palliative and hospice care patients.

  • If you have an urgent emergency for a new patient and cannot reach us, please call your primary care veterinarian or local emergency hospital.


Service Area:

Our regular service area includes locations within the City of Chicago & Near North/North Shore suburbs that are a maximum of 12 miles (or 45 minutes) of our office location. 

  • For locations where rush hour/time of day has a major impact on travel time, appointments may be limited.

  • We may be able to see patients for in-person appointments outside of our regular travel area, but services may be limited, especially in cases of urgent/emergency care. 

  • TeleHealth appointments may be a good option for patients outside of our travel area.

Payment Methods

Payment is respectfully required at time of services. 

  • Cash, Venmo and Zelle/Quickpay are our preferred payment methods. 

  • Major credit cards are also accepted.

  • We are not able to accept Care Credit or other medical care payment programs.

Out of Office Alerts

  • Unless noted, our office is opened regular hours for reception/nursing communications and medication pick-ups. 

  • Dr. Cook is out of the office and unavailable for regularly scheduled appointments, emergency care appointments, and after-hours emergency calls and services.

  • If emergency care is needed, please call or visit your primary care veterinarian or local emergency hospital.
